Steps for Medical Cannabis Certification

Step 1: Physician Certification

Contact Dr. Dodson’s office staff for a certification screening to check if you are a candidate for medical cannabis. Our phone number is 763-588-7099.

 

  • Have your medical records emailed or faxed to our facility at fax number: 1-877-849-3529.
  • Once medical records are reviewed and accepted, you will be scheduled with Dr. Dodson and physician certified.
  • You will be required to provide Dr. Dodson with an email address.
  • If you do not have an email, you will be asked to acquire an artificial email by calling the Office of Medical Cannabis.
Step 2: State Register

You will receive a link using the address you provided to Dr. Dodson from the Office of Medical Cannabis.
The email will contain the link you need to click on in order to register and submit your enrollment application. Wait for your approval email from the MN Dept of Health OMC.

Step 3: Visit a Cannabis Patient Center/Dispensary

You, your parent, legal guardian, or caregiver must visit the cannabis patient center, where a pharmacist will review your account and recommend specific dosage and type. Payment will be made at that time to the cannabis patient center for your medical cannabis medication.

Step 4: Fill Out Your Patient Self Evaluation Form

Before returning for more medical cannabis, you must log in to the registry and fill out the patient self-evaluation form. This must be done each time you need more medical cannabis.

Find Out if You Qualify

Only patients who are legal Minnesota residents and have been diagnosed with one of the following qualifying conditions are eligible to receive this natural, effective medical treatment in Minnesota.

Qualifying Conditions:

  • Alzheimer’s disease
  • Amyotrophic lateral sclerosis (ALS)
  • Autism spectrum disorder (must meet DSM-5)
  • Cancer
  • Chronic motor or vocal tic disorder
  • Chronic pain
  • Glaucoma
  • Inflammatory bowel disease, including Crohn’s disease
  • HIV/AIDS

  • Intractable pain
  • Irritable bowel syndrome
  • Obsessive-compulsive disorder
  • Obstructive sleep apnea
  • Post-traumatic stress disorder (PTSD)
  • Seizures, including those characteristic of epilepsy
  • Severe and persistent muscle spasms, including those characteristic of multiple sclerosis (MS)
  • Sickle cell disease
  • Terminal illness with a probable life expectancy of less than one year*
  • Tourette syndrome
